Guidelines for Cross Regional Referrals

These guidelines enhance the collaborative working relationship between Entry Points across the state to ensure consistent practice and processes are in place for out of region referrals.

Guidelines for Statewide/Multi-area Services and local Entry Points

The purpose of this agreement is to improve our response and enhance a collaborative approach between local Entry Points and Statewide/multi-area services. These guidelines are intended to reduce the challenges a client faces when accessing the homelessness service system, by shifting the responsibility of navigating a complex service system from the client to funded services. A coordinated response and transparent pathways will assist services to make and receive appropriate referrals across the state.

The Opening Doors framework is summarised in three documents:
• this Framework document
• the Service coordination guide for local area service networks
• a Practice guide for workers.

Opening Doors Practice Guide

This guide assists workers who undertake initial homelessness assessment to understand and implement
the practice of Opening Doors. It includes information about using the initial assessment and referral tools,
and will support each worker’s own communication and allow for individual tailoring of the assessment

The purpose of this agreement is to enhance collaborative working relationships between the housing and homelessness service system and Corrections Victoria in order to improve the response to people who are at risk of homelessness when exiting correctional facilities.

This manual has been designed as a Train the Trainer Manual in order to teach you about using the Opening Doors Resource Register as both an Agency Manager and an Agency Officer, facilitated by your Local Area Service Network‟s (LASN‟s) trainers.

This guide will assist local area service networks to develop, implement and maintain their local service
delivery model. It divides the development work into seven stages, and provides:
• information about membership, roles and responsibilities of the local area service networks
• a detailed outline of the tasks for each LASN
• tools and resources to assist each LASN with these tasks.
